III - Working with AI-Informed Patients: Preserving Trust in the Age of ChatGPT
In Part 1 of this series, we examined the dangerous imbalance created when patients are free to use AI while clinicians are banned from doing so. In Part 2, we explored the policy vacuum that allows this asymmetry to persist, leaving Australian healthcare far behind nations like the UK, the US, and Singapore in adopting safe, structured AI use.
Now, in Part 3, we turn to the front line: the consultation room. Here, the impact of these policies — or lack thereof — becomes tangible. Patients are arriving not just with symptoms, but with an AI-generated narrative about what those symptoms mean. For doctors, this is more than a curiosity; it is a new and permanent layer in the clinical encounter.
The New Patient Mindset
Generative AI tools like ChatGPT can now summarise lab results, list likely diagnoses, and outline treatment options. For patients, this feels empowering. For doctors, especially in emergency medicine, it changes the starting point of the conversation.
Some patients arrive confident in an AI-suggested diagnosis and question the need for further tests. Others arrive alarmed by an AI-generated “worst case scenario,” making reassurance harder. In both cases, the doctor must navigate not just the medical facts but the emotional weight of an unseen algorithmic voice.
The Human Advantage
Part 1 argued that banning clinicians from using AI is unsafe. Part 3 shows why: AI cannot pick up on the micro-hesitations, guarded expressions, or subtle contradictions between words and body language that shape a doctor’s decision-making.
In emergency medicine, these cues are more than soft skills — they can alter the course of care. A middle-aged man insisting his chest pain is “just stress” may unconsciously guard his left arm. A teenager with abdominal pain may avoid eye contact when asked about alcohol. These observations, invisible to an algorithm, are often decisive.
The Trust Equation
Part 2 warned that policy neglect could erode patient trust. Here is where that risk becomes real. Patients who have already placed trust in AI may see their doctor’s rejection of it as a dismissal of their own effort to understand their health. The relationship sours before the examination has even begun.
Acknowledging AI’s role can turn this dynamic around:
“That’s an interesting interpretation from ChatGPT — let’s look at your results together and see what applies to your situation.”
By integrating the AI conversation into the clinical encounter, doctors can position themselves as both interpreters and protectors of accuracy.
Equipping Clinicians for the Shift
If healthcare networks ban AI for clinicians while patients use it freely, doctors will remain reactive, not proactive. Training and policy should:
Familiarise clinicians with popular AI health tools so they know what patients are likely to encounter.
Provide strategies for validating a patient’s effort while gently correcting inaccuracies.
Establish clear thresholds for when AI-informed concerns warrant further investigation.
Without these measures, the gap between patient expectations and clinical reality will grow — and so will the trust deficit.

The consultation room of the future is not man versus machine. It is man, patient, and machine in dialogue, with the clinician as the arbiter of safety and relevance. AI-informed patients are here to stay. The question is whether healthcare systems will give doctors the tools and training to meet them on equal ground.
The alternative is not just inefficient care, it is a slow erosion of trust, as patients turn to algorithms for certainty and doctors are left to compete with an entity they are not even allowed to know.
